A classic rehab gets you through withdrawal and teaches you to manage craving. An ibogaine clinic interrupts the withdrawal and resets the craving. Both work on real parts of the problem. Neither was built to reach the part the Bwiti tradition looks at first: why you needed the substance in the first place. Three ways to treat addiction

Addiction lives on three levels at once. In the body: dependence, withdrawal, a brain that has rewired itself around a substance or a behavior. In the mind and the emotions: craving, habits, triggers, and the feelings the substance was managing. And in what the Bwiti tradition calls the spirit: the meaning, the self, the wound or the emptiness that the habit moved in to fill. Each kind of treatment chooses where to put its weight.

Medical detox makes withdrawal safe, and weeks of therapy build skills against craving and triggers. The spirit is touched lightly, through a twelve-step higher power if at all. Aftercare exists, but its strength depends on the program and on you.

What rehab does well, and where it stops

Conventional treatment deserves more credit than it usually gets in the iboga world. Medical detox has made alcohol and benzodiazepine withdrawal safe. Medications for opioid use disorder, buprenorphine and methadone, cut the risk of dying by overdose dramatically. Cognitive behavioral therapy, contingency management and mutual-help groups have real evidence behind them. And insurance often pays.

Where it stops is the place many of our guests arrive from. The U.S. National Institute on Drug Abuse estimates that 40 to 60 percent of people treated for addiction relapse, a rate it compares to other chronic illnesses such as hypertension and asthma. Withdrawal lasts days; craving can last months; and the reasons behind the use, a trauma, a grief, a life that feels empty, are often talked about for weeks without ever being met. People describe leaving rehab with good tools and the same hole inside.

What ibogaine clinics do, and what they leave out

Ibogaine clinics, most of them in Mexico, have done something important: they showed that one long night can interrupt opioid withdrawal and quiet craving for weeks, something no conventional detox does. What most of them leave out is everything around the molecule. Ibogaine was taken out of a ceremony and put into a clinic, and the clinic usually treats it as a detox drug: a dose, a bed, monitors, an eye mask, and a long night that is often lived alone. Visions of childhood, of people who have died, of the moment everything went wrong, arrive with no one trained to hold them and no tradition to give them a place. The next day is recovery. A few days later you fly home with a quiet craving and an experience nobody helped you understand. When the craving comes back, there is nothing underneath it that has changed.

That is not a criticism of ibogaine. It is the reason we work the way we do.

Detox removes the substance safely and therapy works on the habits around it. The trunk, the craving, is managed rather than reset, and the roots are discussed more than met. When stress returns, the tree can grow back from below.

What Bwiti Life does differently

We did not invent a new method. We put back together two things that were separated: the medical safety of a good clinic, and the tradition that iboga comes from.

  • The emotions, worked with. A processing day after each ceremony, a psychologist on the team, a group of eight people who went through the same nights, and a cenote to rest in.

When rehab is the better choice

We would rather lose a booking than have someone in the wrong place. A conventional program, or a medication, is the better path when:

  • You are stable on buprenorphine or methadone and it is keeping you alive and well. Medication for opioid use disorder saves lives; changing it is a decision for you and your doctor, not a retreat.
  • You have a heart condition, an abnormal ECG that cannot be cleared, or a history of psychosis or bipolar disorder.
  • You are in acute alcohol or benzodiazepine withdrawal, which needs a medical detox now.
  • You need a long residential stay away from home, or insurance coverage.
  • You are under 18. Our retreats are for adults.

Questions to ask any ibogaine clinic

Whether you choose us or someone else, ask these before you pay. A good provider answers all of them without hesitation.

  • Is an ECG and full bloodwork required before I travel, and who reviews it?
  • Is a physician in the room, or on the premises, for the whole night? Is the heart monitored continuously? Is there IV access?
  • What exactly will I be given: ibogaine HCl, a total alkaloid extract or whole root bark? How is the dose decided?
  • How do you handle methadone, buprenorphine, benzodiazepines, alcohol, kratom and stimulants before treatment?
  • Who sits with me during the night, and what is their training?
  • What happens the day after, and the week after? Who helps me understand what I saw?
  • What support do I have after I fly home, for how long, and with whom?

Questions people ask

Is ibogaine better than rehab?

They do different things. Ibogaine can interrupt withdrawal and reset craving in a way rehab cannot; rehab offers weeks of structured therapy, insurance coverage and proven medications. There are no head-to-head trials. For many people the strongest path combines both: the window iboga opens, and the skills and support that keep it open.

What is ibogaine comparable to?

Nothing quite. It is sometimes grouped with psilocybin, ayahuasca or ketamine, but its long, lucid night, its effect on opioid withdrawal and its cardiac risks set it apart. Read Iboga vs psilocybin, ketamine and MDMA and Iboga vs ayahuasca.

Why do people relapse after rehab or ibogaine?

Usually because the person goes home to the same cues, the same stress and the same reasons, with less support than they had inside. After rehab the craving is still there; after an ibogaine clinic the craving may be gone but the reasons are not. That is why we put as much weight on the roots and on the three months after as on the night itself.

Can ibogaine heal addiction?

Not by itself. Observational studies show reduced withdrawal and craving and long periods of abstinence for many people, but no medicine cures addiction alone. We describe iboga as a strong opening, not a cure. Read Ibogaine success rates.

Does ibogaine heal trauma?

A 2024 Stanford study of veterans with traumatic brain injury found large reductions in PTSD, depression and anxiety a month after ibogaine. Guests describe seeing traumatic memories with a new distance. Whether that becomes healing depends a great deal on how the experience is held and integrated, which is the heart of the Bwiti approach. Read Iboga for PTSD and trauma.

Does ibogaine change you?

It does not change who you are; guests more often say it showed them who they were under the habit. What changes is perspective, and for a while, craving and mood. Lasting change comes from what you do with that in the following months.

Does insurance cover ibogaine or iboga treatment?

No. Ibogaine is not an approved treatment in the United States or Canada, and insurers do not cover it or iboga retreats abroad. Some guests use their insurance for the laboratory tests before travel or for therapy afterward. Read Cost and what is included.

What is the difference between iboga and ibogaine?

Iboga is the plant, whose root bark contains a family of alkaloids. Ibogaine is the best-studied of them, usually given in clinics as purified ibogaine HCl. We use the whole root bark inside the Missoko Bwiti tradition, under medical supervision. Read Iboga vs ibogaine.
